School & sports physicals
Before your child attends school or camp, or participates in a sport, he might be required to undergo a physical exam. This exam is aimed at making sure your child is physically capable of safely participating in the actiivity and does not have any underlying health issues that could be a cause for concern.
If your child needs a form filled out and has a well-child visit scheduled, please bring the form to your child's appointment. There is no extra charge for filling out required forms during well-child visits.
If your child needs a school, camp or sports physical form filled out and has already had an annual well-child checkup, our office might have the information needed to complete the required form without a separate visit. There is no charge for having school, sports or camp physical forms filled out between well-child visits. Please allow at least two days after you have gotten these forms to our office for us to complete them.
If a physical requires tests not typically included in a well-child checkup, a separate appointment might be necessary. Some insurance plans do not cover these appointments, so please check with your insurer before your appointment if you are concerned.
